If the bidder is an institution, it should have a legally valid legal person certificate of the institution. Scanned copy of the original shall be provided at the time of bidding (the original is available for inspection). If the bidder is a branch company, it shall have a legally valid business license and authorization letter of the superior legal entity. Only one branch company and the superior legal entity can participate in the bidding; if they participate in the bidding at the same time, the bidding will be invalid. Scanned copy of the original is required to be provided at the time of bidding (the original is available for inspection).If the bidder is an overseas registered company, it is necessary to provide a valid certificate of company registration.*.The bidder shall be the manufacturer of the product under tender. The tender shall not be accepted from agents or traders. The wholly-owned sales company or holding sales company of the manufacturer of the goods tendered, as well as the parent company of the manufacturer of the goods tendered are accepted to participate in the bidding, and such company is regarded as the manufacturer, and the bidding shall be provided with the corresponding supporting documents. Only one bidder from the same manufacturer is allowed to participate in the bidding, otherwise all related bids will be rejected.*.The manufacturer of the products submitted by the bidder shall have a valid GB/T***** (ISO****) quality system certification or API Q* certificate, which can be verified on the website of China Certification and Accreditation Administration (https://www.cnca.gov.cn/). Note: If the quality management system certification provided by the manufacturer of the bidding goods is issued by a foreign third-party certification body, the website query is not applicable.二、業(yè)績(jī)要求：****年*月*日至投標(biāo)截止日（以合同簽署時(shí)間為準(zhǔn)），投標(biāo)人所投產(chǎn)品的制造商應(yīng)具有至少*個(gè)合同或訂單的被動(dòng)式封隔器的供貨業(yè)績(jī)。投標(biāo)人須按規(guī)定格式提交業(yè)績(jī)表，并提交相關(guān)業(yè)績(jī)證明文件。業(yè)績(jī)證明文件包括但不限于：*）合同掃描件和*）到貨驗(yàn)收材料。投標(biāo)人所提交的業(yè)績(jī)證明文件必須至少體現(xiàn)以下內(nèi)容：合同簽署時(shí)間、合同簽署頁(yè)（應(yīng)有雙方簽署或蓋章）、制造商名稱(chēng)、貨物名稱(chēng)及到貨驗(yàn)收材料（至少包含：貨物驗(yàn)收單或結(jié)算清單或?qū)?yīng)的發(fā)票（如有清單須提供，須體現(xiàn)發(fā)票號(hào)；發(fā)票需包含合同號(hào)或訂單號(hào)或貨物名稱(chēng)或買(mǎi)方賣(mài)方信息，且與合同或訂單一致）或買(mǎi)方接收證明或調(diào)試驗(yàn)收證明。若業(yè)績(jī)?yōu)橛唵危杼峁⿲?duì)應(yīng)的協(xié)議掃描件、訂單掃描件及訂單相應(yīng)的到貨驗(yàn)收材料。貨物驗(yàn)收單或結(jié)算清單至少應(yīng)有合同甲方簽字或蓋章。未提交業(yè)績(jī)證明文件，或所提供的業(yè)績(jī)證明文件無(wú)法體現(xiàn)滿(mǎn)足上述業(yè)績(jī)要求的，均視為無(wú)效業(yè)績(jī)。From January *, **** to the deadline for bidding (subject to the time of contract signing), the manufacturer of the products bid by the bidder shall have at least * contracts or orders for the supply of Grout Seals.Bidders must submit a performance table in the prescribed format and submit relevant performance proof documents. If the performance is an order, the corresponding agreement scan, order scan, and delivery and acceptance materials for the order should be provided.
